Can Vitamin D Prevent Sports Injuries? What the Latest Research Shows
If you've spent any time in the endurance sports or strength training world, you've probably heard that vitamin D can help prevent injuries.
The idea makes sense.
Vitamin D plays a critical role in bone health, muscle function, and calcium absorption. Athletes with low vitamin D levels are also more likely to experience stress fractures and other musculoskeletal injuries in observational studies.
But does taking a vitamin D supplement actually reduce your injury risk?
A new randomized, placebo-controlled trial published in the British Journal of Nutrition set out to answer exactly that question.
The results were surprising.
While vitamin D supplementation successfully restored healthy vitamin D levels in nearly all participants, it did not significantly reduce overuse musculoskeletal injuries during military training or over the following three and a half years.
So does that mean vitamin D doesn't work?
Not at all.
Here's what the study found—and what it really means for athletes, runners, and anyone trying to stay healthy and injury-free.
Why Vitamin D Is So Important
Vitamin D is often called the "sunshine vitamin" because our bodies produce it when our skin is exposed to sunlight.
Its best-known role is helping the body absorb calcium, but vitamin D does much more than support healthy bones.
Research has shown that vitamin D plays important roles in:
- Bone mineralization
- Muscle function
- Immune health
- Inflammation regulation
- Recovery from exercise
- Cellular signaling
Because of these wide-ranging effects, many athletes routinely test their vitamin D levels or take supplements, especially during the winter months when sunlight exposure is limited.
Why Researchers Thought Vitamin D Might Prevent Injuries
The theory didn't come out of nowhere.
Several observational studies have reported that athletes and military personnel who suffer stress fractures or muscle injuries often have lower vitamin D levels than those who remain injury-free.
That led scientists to wonder:
If low vitamin D is associated with more injuries, could correcting vitamin D levels actually prevent injuries?
It's an important question—but one that observational studies can't answer.
Just because two things are associated doesn't mean one causes the other.
That's why randomized controlled trials are so valuable.
The New Study
Researchers from the United Kingdom recruited healthy young British Army recruits beginning one of the most physically demanding training programs in the military.
This created the perfect environment to study overuse injuries because military training places tremendous stress on bones, muscles, and connective tissues.
Participants were randomly assigned to receive either:
- Oral vitamin D3 supplementation plus simulated sunlight exposure
- Placebo tablets plus placebo light exposure
The intervention lasted 12 weeks, with researchers tracking injuries throughout training and continuing to monitor participants for 3.5 years after enrollment.
The Good News: Vitamin D Worked Exactly as Expected
First, the supplementation successfully did what it was supposed to do.
Researchers found that vitamin D:
✅ Restored vitamin D sufficiency in approximately 95% of participants
✅ Increased blood concentrations of vitamin D metabolites
✅ Lowered parathyroid hormone (PTH), an indicator of improved vitamin D status
In other words, the supplement effectively corrected vitamin D insufficiency.
The Surprise: Injuries Didn't Decrease
Here's where things became interesting.
Despite improving vitamin D status, researchers found no clear evidence that supplementation reduced lower-body overuse injuries.
Compared with the placebo group, vitamin D supplementation did not significantly reduce:
- Overuse musculoskeletal injuries
- Bone stress injuries
- Lost training days
- Reduced-duty days
- Medical discharge due to injury
- Long-term injury rates during military service
For a supplement widely believed to prevent injuries, that was an unexpected result.
Why Didn't Vitamin D Reduce Injury Risk?
This is where the findings become more nuanced.
The researchers weren't surprised that vitamin D improved blood levels—but they offered several possible reasons why that improvement didn't translate into fewer injuries.
1. There Weren't Enough Injuries
Although military training is demanding, the total number of injuries observed during the study was relatively small.
That means the trial may not have been large enough to detect modest reductions in injury risk.
The authors specifically noted that smaller beneficial effects cannot be ruled out because of the limited number of injury events.
2. Vitamin D May Not Work Alone
One particularly interesting point comes from previous research.
Earlier studies that did report fewer stress fractures typically supplemented both vitamin D and calcium, not vitamin D alone.
Calcium helps maintain blood calcium levels during exercise and may reduce bone resorption, especially during high-impact activities.
Researchers suggest this combination may be necessary to meaningfully reduce injury risk, particularly if dietary calcium intake is low.
3. Healthy Young Adults Are Different
The participants in this study were young, otherwise healthy military recruits.
Results may differ in:
- Older adults
- Female athletes
- Individuals with severe vitamin D deficiency
- People with darker skin living at northern latitudes
The researchers emphasized that more work is needed in these populations before broad conclusions can be made.
4. Injury Prevention Is Complex
No single nutrient can overcome poor recovery, excessive training loads, inadequate sleep, or insufficient calorie intake.
Injury risk depends on dozens of interacting factors, including training volume, biomechanics, nutrition, genetics, and recovery habits.
Vitamin D is just one piece of that puzzle.
Does This Mean Vitamin D Isn't Worth Taking?
Not at all.
One of the biggest mistakes people make when reading nutrition headlines is assuming that if a supplement doesn't improve one outcome, it has no value whatsoever.
That's not what this study showed.
Vitamin D remains essential for maintaining normal bone health, supporting muscle function, aiding calcium absorption, and contributing to a healthy immune system. In this trial, supplementation successfully restored vitamin D sufficiency in almost all participants and lowered parathyroid hormone (PTH), indicating improved vitamin D status.
The key takeaway is simply this:
Correcting vitamin D levels didn't automatically translate into fewer overuse injuries in this group of healthy young military recruits.
That's a very different conclusion than saying vitamin D isn't important.
What About Stress Fractures?
Stress fractures are one of the injuries most commonly linked to low vitamin D.
Previous observational studies have found that athletes and military personnel with lower vitamin D levels are more likely to experience bone stress injuries.
However, this study wasn't able to confirm that supplementing with vitamin D alone reduced stress fracture risk.
One reason is simple: there were very few bone stress injuries during the trial, making it difficult to detect meaningful differences between groups. The researchers acknowledged that the study wasn't powered to analyze these injuries separately.
So while vitamin D deficiency remains a concern—especially for athletes at higher risk of stress fractures—this study suggests that supplementation alone isn't a guaranteed solution.
What Should Athletes Do Instead?
If your goal is staying healthy and reducing injury risk, this study reinforces a broader lesson:
There isn't a single "magic" supplement that prevents injuries.
Instead, athletes should focus on the fundamentals:
✔ Maintain Healthy Vitamin D Levels
If testing shows you're deficient, work with your healthcare provider to restore normal levels through appropriate sun exposure, diet, or supplementation.
✔ Eat Enough Calcium
Vitamin D helps your body absorb calcium, but it can't do its job if calcium intake is too low.
Previous military research showing fewer stress fractures used vitamin D combined with calcium, suggesting both nutrients may be important for bone health.
✔ Prioritize Protein
Adequate protein supports muscle repair, tendon health, and recovery after training.
✔ Train Progressively
Rapid increases in mileage, intensity, or training volume remain one of the biggest contributors to overuse injuries.
✔ Recover Well
Sleep, hydration, total calorie intake, and smart recovery habits are all critical pieces of the injury-prevention puzzle.
No supplement can compensate for poor recovery.
The Bottom Line
Vitamin D continues to be one of the most important nutrients for overall health, particularly for supporting bones, muscles, and the immune system.
But according to this well-designed randomized controlled trial, restoring vitamin D sufficiency alone did not significantly reduce overuse musculoskeletal injuries in healthy young military recruits.
That doesn't mean vitamin D isn't valuable.
It simply reminds us that injury prevention is complex.
Maintaining healthy vitamin D levels should be viewed as one part of a comprehensive strategy that also includes resistance training, proper nutrition, sufficient calcium intake, gradual training progression, and adequate recovery.
